A man and a teenager have been taken to hospital after a crash on a busy dual carriageway near Balsall Common, Solihull.
The pair were in a Nissan Serena car which collided with a Mercedes E280 on the A452 Kenilworth Road between the Stonebridge and Meriden roundabouts.
The Nissan left the road and crashed through a wall and into some trees.
A West Midlands Ambulance Service spokesman said: "The vehicle had come to a stop further up the road from where it had gone through the brick wall.
"There was a hedge of conifers on the other side of the wall. It was facing the wrong way and there was significant damage to the passenger side of the vehicle.
"There were two casualties who were lying some distance from the vehicle though it was unclear to the crews how they got there.
"One man, who is 28, had multiple serious injuries to his shoulder, leg and foot all on the left side. The other youth, who is 17, had a possible broken jaw and crews were concerned that he had suffered internal injuries.
"Both were immobilised using neck collars and spinal boards before being taken to Heartlands Hospital - the 28-year-old was due to undergo surgery."
Anyone who witnessed the collision has been urged to call the road policing unit at Solihull on 0845 1135000.
